Proceed to Registrar’s Office (Window 2) for checking of initial requirements.

Pay admission or testing fees at the cashier (Window 5)

Proceed to Student Development Center (SDC) for scheduling of admissions test and interview.

Feedback and details for enrollment will be given for those who will pass the admissions test and interview.

Frequently Asked Questions

What is the age requirement for Admission in specific levels?
Applicants must meet the age limits set for the following levels at the start of the school year:
  • Prekindergarten – must be 4 years old on or before October 31
  • Kindergarten – must be 5 years old on or before October 31
  • Grade 1 – must be 6 years old on or before October 31
What is the grade requirement in the applicant’s official school record?

Kindergarten to Grade 9 and Grade 11 Applicants are required to submit a Certified True Copy of the Report Card with Learner Reference Number (LRN) of the present grade level. The report card must at least bear the grades of the first two grading periods. Only applicants with no failing grades will be accommodated.

What are the requirements for application?
  1. Certified True Copy of the Report Card (Form 138) with Learner Reference Number (LRN) reflecting at least grades of the two grading periods for Kindergarten to Grade 9 and Grade 11 applicants. Only applicants with no failing grades will be accommodated.
  2. Original copy of Birth Certificate issued by Philippine Statistics Authority (PSA)
  3. 2 copies of recent colored 2x2 photo with white background
  4. Certificate of Good Moral Character from the last school attended (for Grades 4 to 6, Junior and Senior High applicants only)
  5. Recommendation Letters: (Use the Recommendation Forms provided and submit in a sealed envelope with the signature of the person recommending across the flap. This may be submitted on the day of the admission test.)
    a. From the Principal/Guidance Counselor/Class Adviser (Grades 1 – 9 and Grade 11 applicants only)
    b. From the Values Education Teacher/Pastoral Ministry Coordinator/School Chaplain/Sister Adviser (for Junior and Senior High applicants only)
What are other requirements required upon enrollment?
  1. Photocopy of the Baptismal Certificate
  2. Photocopy of Marriage Contract of Parents (if applicable)
  3. Medical Certificate
  4. Alien Certificate of Registration (ACR) if applicable
  5. Certificate of ESC Grant from previous school
